Epidermal whitening or gray discoloration

Immediate epidermal blanching, whitening, or gray discoloration is an adverse endpoint. It suggests excessive thermal injury to the epidermis and increases the risk of blistering, scarring, and pigmentary complications.

Treatment should not be escalated simply to make the lesion appear more dramatically injured.

Excessive purpura or vessel rupture

Purpura is expected, but unnecessarily intense or confluent purpura may reflect vessel rupture and extravasation rather than controlled coagulation.

Why shorter intervals are not the default

Intervals of 3–4 weeks are described for some superficial proliferating vascular lesions, but that schedule should not automatically be applied to port-wine stains.

Similarly, a 6–8-week follow-up may be useful for clinical review, photography, and healing assessment, but the next treatment should be performed only when the skin has recovered and the residual lesion can be assessed reliably.

Treat the endpoint, not a fixed number

A nominal fluence or pulse duration cannot replace direct observation of the skin response.

The target is controlled vascular change with preservation of the epidermis—not the highest possible fluence or the most intense purpura.

More aggressive treatment is not always better
